Lets tackle the training first, there are many more factors to consider such as:
Muscle balance....Take a look around next time you are at your local gym, a great source of amusement to me are the people that focus solely on their chest and biceps. "Think Johnny Bravo Cartoon Character". What about the legs and the other 8 muscle groups?? They are at serious risk of injury as their bodies are so out of balance.
Ego training is also worth a mention here, the guys lifting as much weight as possible, using terrible technique and putting themselves at risk of injury.
Don't get me wrong I do sort of enjoy the entertainment but on the serious side these guys are heading towards a whole host of future physical problems and some are sadly irreversible.
If you get the repetition range wrong, you will look more like a powerlifter than a body builder.
You will also need advice regarding your nutrition. Most people who are underweight do not eat enough food in the first instance, this isn't however to say that they need to over indulge in anything that they can get their hands on, it has to be the correct food to increase muscle, not clog the arteries with fat.
